Can you miss it?
Yes. If you happen to be playing on Extreme mode and change the difficulty at any point in time, you will lose your chance at obtaining the achievement. You must stay on Extreme difficulty from start to finish.
How do you complete A Passion for Extreme?
Extreme difficulty is the most…well, extreme difficulty the game has to offer. As such, this achievement only requires you to complete the main questline from the beginning to end. You don’t need to do any side quests. You don’t need to go out of your way to battle the mini-bosses of the world.
If things get rough, just remember to keep a straight line from start to finish.
